Mae Llesiant Delta Wellbeing, Cwmni Masnachu Awdurdod Lleol sy'n eiddo i Gyngor Sir Caerfyrddin, yn darparu technoleg gynorthwyol a monitro rhagweithiol i gefnogi pobl hŷn a phobl agored i niwed i fyw'n annibynnol gartref am gyfnod hwy.

Ers ei sefydlu yn 2018, mae wedi ehangu ei wasanaethau i ddarparu ystod o atebion Gofal trwy Gymorth Technoleg yn y sector iechyd a gofal cymdeithasol. Mae'n gweithio'n agos gyda phartneriaid iechyd a gofal cymdeithasol lleol a chenedlaethol i ddarparu un pwynt mynediad ar gyfer iechyd a gofal cymdeithasol yn Sir Gaerfyrddin, ac yn darparu gwasanaethau arloesol i gefnogi cleifion sy'n gadael lleoliadau acíwt yn ogystal ag yn y gymuned.

Mae Delta i Chi yn rhoi tawelwch meddwl i chi gyda larwm personol a monitro ddydd a nos felly mae help yno bob amser pan fydd ei angen arnoch. Os oes rhywbeth yn mynd o'i le, pwyswch y botwm a bydd ein tîm yn ateb eich galwad. Mewn ardaloedd penodol, gallwn hefyd anfon ymatebwyr hyfforddedig i'ch cartref i helpu yn bersonol.
